During your hospital stay, you can count on the care management experts across our health system to serve as a bridge between you, your family, and the complex medical system. They include licensed social workers and registered nurses with varied experience and education in care management. They provide various services to smooth the course of treatment and recovery, both during your hospital stay and beyond.

Care management begins with evaluating your physical, psychosocial, and emotional needs. If your care manager determines that further care management intervention is needed, goals are established with you and your family and monitored routinely. This process starts upon admission to the hospital and is ongoing until an appropriate, individualized plan for discharge has been completed.

Care Plan Development: Care management experts work with your doctors and nurses to create a personalized plan that addresses your medical needs, considering your overall health and preferences. 

Education and Support: Care management experts assist you and your family by helping to identify barriers to wellness, supporting and guiding you through episodes of illness, and linking you with needed resources.

Discharge Planning: Care management experts work with you and your family to develop a personalized discharge plan that meets your unique needs. They’ll provide the resources and information you and your loved ones need to feel confident and comfortable—regardless of whether you are discharged home or to a post-acute care facility.
