Clinical Notes, the newest My Patient Connect feature, provides access to important information about your office visits and hospitalizations. The information available via Clinical Notes can help you remember your treatment plan and partner with your providers to ensure you have the best outcomes.
Clinical Notes are available for visits occurring November 2, 2020, and forward based on patient portal configuration and design. Clinical Notes for visits prior to November 2, 2020, can be requested through Concord Hospital Health Information Management Services.
Access Clinical Notes via the Health Record navigation menu in My Patient Connect. If you haven't already enrolled in My Patient Connect, now is an excellent time to do so.
