When returning home following an acute hospital stay is not medically appropriate, and you need short-term subacute skilled care and rehabilitation services, our Skilled and Restorative Care program at Concord Hospital - Franklin serves as a bridge between hospital and home. With optimal healing known to occur at home, our Skilled and Restorative Care program empowers you to regain your strength, independence, and confidence, facilitating a swift and safe return home. Whether regaining mobility, managing a chronic condition, or simply learning to perform daily tasks efficiently, our team of experts will develop an individualized care plan to meet specific goals and expectations for your stay.

What You Can Expect

Full range of services: Physical therapy, occupational therapy, speech and language therapy, respiratory therapy, on-site laboratory services, and access to providers 24/7 are available to complement the care provided.

Seamless care: Admission into our program allows you to remain within our health system while receiving short-term subacute nursing and rehabilitative care, avoiding transitions to other facilities. With this continuity of care comes the benefit of one medical record. Program staff have access to the electronic medical record system used in Concord, Franklin, and Laconia and to providers with privileges at all three hospitals.

Interdisciplinary care: A team of professionals, including nurses, physicians, advanced practice providers, therapists, social workers, care managers, and activity professionals, work together to address your individual needs.

Personalized care: Treatment plans are tailored to your specific requirements. After admission, your care team will meet with you, your family, and/or your caregiver to develop an individualized care plan to meet specific needs and goals for your stay.

Short-term stay: The average stay is between seven and fourteen days. All rooms are private; you will not have a roommate during your stay.

How to Access Our Skilled and Restorative Care Program

Accessing our Skilled and Restorative Care program is easy and convenient. During your hospital stay, a referral will be set up. If you’re a patient at Concord Hospital - Franklin, your care manager will communicate with the hospital's referral specialist. You will be able to transfer from acute care to the program’s subacute/skilled care without having to leave your room. If you’re a patient in Concord or Laconia, your care manager will communicate with the referral specialist in Franklin to arrange your transfer to the Skilled and Restorative Care program.
