The home evaluation is a pre-surgical home safety assessment developed to prepare the patient and caregiver for pre-and post-surgical needs, assist in discharge planning and improve quality, continuity and efficiency of services along the continuum of care in rehabilitation.
The focus of the home evaluation is on pre-surgical assessment of you and your home environment. The physical therapist may make helpful suggestions to ensure that you have appropriate furniture to sit in after surgery, plus suggestions on how to make your home as safe as possible. They will talk to you and your caregivers about available resources and what may be expected from your caregivers.
