Concord Hospital Health System Receives Excellence in Patient Safety & Quality Improvement Award

Concord Hospital Health System (CHHS) has been honored with the Excellence in Patient Safety & Quality Improvement Award at the New Hampshire Hospital Association and Foundation for Healthy Communities Annual Meeting.

This award recognizes healthcare teams that demonstrate a commitment to quality care, lead quality improvement initiatives, promote transparency to improve health care, and achieve better outcomes through a strong culture of safety. CHHS earned this distinction through its Patients First initiative, a strategic priority focused on providing exceptional care across six domains of healthcare quality: safe, timely, effective, efficient, equitable, and patient-centered.

An eight-member multidisciplinary team led efforts across the health system to enhance patient safety outcomes, reduce health disparities, and improve transparency for care teams, patients, families, and community stakeholders.
