Concord Hospital is a charitable organization which exists to meet the health needs of individuals within the communities it serves.
Rehabilitation Services of Concord Hospital provides comprehensive physical and occupational therapy to help patients regain their functional independence by
allowing them to return to normal daily self-care, work and leisure activities. Our outpatient services are offered at six sites including CH Medical Offices
North, the Center for Sports Medicine and Orthopaedic Rehabilitation, CH Medical Offices at Horseshoe Pond, CH Medical Offices East, Warner and Pembroke.
