Patient Connect is a secure online service that allows you to keep your personal health information up-to-date and correspond with your Concord Hospital Medical Group (CHMG) practice anywhere you have Internet access. With Patient Connect, you can send a message to your provider's office, request an appointment or medication refill and much more.

I have received lab test results from my CHMG provider, via secure e-mail, or I have accessed Patient Connect previously.

First Time User

Step 1: Enroll

If you are at least 18-years-old and a patient at a participating practice you can request a One-Time Registration Code to enroll in Patient Connect. Once you receive your Registration Code, you can register online.
